DESCRIPTION POSITION TITLE: Physician Department: Adult REPORTS TO:
DR. LETICIA UWEDOJEVWE Date: 05/2022 SUMMARY OF POSITION A Doctor,
or Physician, will be responsible for promoting the health and
safety of our patients by providing proactive treatment and
diagnosis. His/her duties include speaking with patients to address
their health concerns, conducting physical examination to ensure
our patients--- health and administering vaccinations for common
illnesses or allergies. Physician shall provide outpatient primary
health care to acute and chronically ill stable adult patients.
ESSENTIAL DUTIES 1.- INITIAL EVALUATION: The physician as the
Primary Care Provider (PCP) who has been assigned to the patient,
generally on the basis of Patient choice or rotation shall see the
patient at the earliest opportunity. His/her responsibilities are
to: Take care of the patient---s immediate medical needs. Undertake
medical evaluation, including history and physical examination,
laboratory, X-ray, referrals, or other diagnostic studies. Evaluate
the patient and develop a problem list. Develop treatment
objectives and plans. Initial treatment orders and procedures.
Educate and counsel patients to promote positive health behaviors
and diseases prevention. Work closely with clinical staff in
coordination and obtaining needed services for patients and
families. Consult with the Director of Adult Medicine as necessary.
Perform additional duties as assigned. 2.- CONTINUING CARE: The PCP
will make every effort to follow through personally with the
patients he/she evaluated initially. Patients will be asked to
return for further services as often as necessary and by
appointment whenever possible. By the first visit, the PCP will
complete and maintain a problem list to be keep in each patient---s
medical record. 3.- RECORDING OF PATIENT---S PROGRESS, PROCEDURES,
AND OTHER MATTERS: A visit will be recorded, and the chart note
accepted in the ECW in same day of the appointment finish. Notes
should: Reflet changes in a patient---s condition and other
significant information. Include documentation of pertinent
history, review of systems and physical exam Indicate studies
performed and that results are reviewed and discusses with the
patient Reflect provider---s though processes as to the patient---s
problem, diagnosis, and treatment. Document the need for continuing
services and the appropriateness of diagnostic evaluations and
treatment plans. Include referrals to specialty clinic services as
determined to be medically necessary Specify medications prescribed
by the provider Reflect any pertinent attitudes or behavior on the
part of the patient or family towards any aspect of care. Include
notation of the need for routine and preventive services such as
screening tests, periodic laboratory examinations related to
chronic disease care, and immunizations. Results (including but not
limited to labs, radiology results, consult notes, outside records)
received by the provider should be attended to within 1 day, with
acutely abnormal result attended to more urgently. On Call notes
should be entered into the patient---s chart in the same day. Use,
protects, and discloses EPFMC patients--- protected health
information (PHI) only accordance with Health Insurance Portability
and Accountability Act (HIPPA) standards. 4.- PHYSICAL PATIENT
COMMUNICATION: The patient shall be informed by the PCP as to the
general nature of his/her medical problem, the general prognosis
and the nature and purpose of the treatment and procedures that are
contemplated. Appropriate instructions in self-care shall be given
by the PCP and/or the nursing staff to the patient and documented
int patients medical records. Physician interactions with patients
and families are to be always courteous and professional 5.-
COMPLIANCE WITH PRODUCTIVITY AND DOCUMENTATION STANDARS Ensure
productivity standards are met. Goal 15 --- 20 patients to be seen
in an average 8- hours day. Complete all documentation, coding and
data collection meet schedule and accuracy requirements for ECW
data collection purposes. 6.- SUPERVISION: While in clinic,
physician is expected to provide supervision and consultation to
mid-levels providers, NP, interns, and students in training as
needed. EQUIPMENT & SOFTWARE OPERATION This incumbent in this
position may operate any/all of the following equipment: Telephone,
cell phone, fax, scanner, computer, printer and related equipment,
copy machine, audio-visual equipment, Computer software may include
any or all of the following: Microsoft office, Internet Explorer or
comparable programs, Adobe reader and E-Clinical Work (ECW) For
E-Clinical Work system we can train POSITION REQUIREMENTS: These
specification are general guidelines based on the minimum
experience normally considered essential to the satisfactory
performance of this job. Individual abilities may result in some
deviation from these guidelines. TO PERFORM EFFECTIVELY IN THIS
POSITION, THE INCUMBENT MUST HAVE: 1.- Must have a current license
to practive medicine in the State of California. 2.- Board
Certified/Board Eligible in Internal Medicine or Family Practice
3-Must have a current DEA License 4.- Must be bilingual
English/Spanish 5.- Must possess a current CPR Certification 6.-
Must be able to work in interdisciplinary team setting 7.-
Effective communication and interpersonal skills with patients and
families 8.- Experience in working with Electronic Medical Records,
preferred 9.- Timely completion of continuing education necessary
to meet state licensure.
