Position: Registered Nurse – Home Health Unit: RN Home Health
(3504.729000.27022) Location: 200 W Center St, Ste 200C, Anaheim,
CA 92805 Coverage Area: Orange, Villa Park, Yorba Linda, Anaheim
Hills & surrounding areas Start Date: July 14, 2025 Contract
Length: 13 Weeks Shift: 5x8-Hour Days | 8:00 AM – 4:30 PM (No 4x10s
or 3x12s) Weekend Requirement: Every other weekend Patient Ratio:
1:1 (up to 6 visits/day or 30 productivity points) Guaranteed
Hours: Facility may cancel 1 shift per pay period (biweekly) You’ll
provide skilled nursing care in a home health setting with a focus
on high-quality patient care, safety, and independence at home.
Travel between patients is local and efficiently scheduled. Oasis
charting and Epic EMR are used. Mileage is reimbursed at the IRS

Experience: • Minimum 1 year of skilled home health experience –
Required • OASIS experience – Required (must note if not
experienced) • First-time travelers – Not Accepted Certifications:
• BCLS (AHA) – Required • CA RN License – Active or pending at
submission Other Requirements: • Must have reliable personal
transportation • Open to locals within 50-mile radius (same rate
applies) • No on-call or floating between units • No block
scheduling Systems Used: • Charting: EPIC OASIS • Scrubs: Any color
