Job Description:

Position Summary:

Under the supervision of the Mountain Health Community Center Program Supervisor the Resource Coordinator has oversight of the Food Pantry, other food resources/coordination, and Senior Home Delivery meal programs.

Essential Functions of the Job:

* Shares responsibility for coordinating the acquisition and distribution of food from the various project partners and other supportive entities. Feeding San Diego Mobile Food Pantry and San Diego Food Bank. * Maintain Resource Directory, electronically of agencies and contacts, which may be of assistance to Community Center. * Assist the Program Supervisor with the oversight, distribution, and stocking of MHCC's Food Pantry and other food events. * Maintains accurate record of meals, vehicle, mileage, and temperature logs for Senior Home Delivery program. * Prepare daily, weekly, and monthly reports of all activities and programs. * Assist clients in making contact for needed services by way of periodic surveys, monitoring of the suggestion box, a personal interaction, and other means. * Oversee that outreach services are conducted and that the community is aware of the programs and resources that are available at the Community Center. * Schedule and make quarterly visits/assessments with the seniors within the Home Meal Delivery Program. Conducts initial and annual intakes for initial/continual use of AIS Program and check for senior's wellbeing. * Trains HMD Drivers on their daily tasks, including record keeping, obtaining resources for seniors, and be the lead for on reporting unusual circumstances with any of the homebound seniors. * Prepare Meal Delivery Route paperwork for Home Meal Delivery Drivers ensuring it contains clear number of meals, address, client names, phone numbers, and special instructions. * Be willing to work evenings and weekends as needed.

* Adheres to all applicable SYHealth policies, procedures, protocols and best practices. * Demonstrates effective verbal and written communication skills in collaboration with teams to achieve quality, excellence, harmony and problem-solving to obtain stated goals and objectives. * Demonstrates active listening skills, respect and a positive "Can Do" attitude. * Enhances professional growth and development through participation in educational programs, reading literature to keep up-to-date related to Healthcare laws and Leadership, attending in-service trainings and workshops to improve upon knowledge and skillset. * Attends meetings, as required, and participates on committees, as directed. * Demonstrates professional behavior standard etiquette, courtesy, mutual respect, confidentiality and treats every patient and employee with dignity in every interaction. * Exhibits a high level of integrity, ethics, transparency, leadership style and trust. * Promotes and fosters respect for patient privacy by maintaining confidentiality on all patient EHR data and abides by both the HIPAA Privacy and Security rules. * Exhibits excellent presentation skills, confidently speaks up, follows the appropriate chain of command and demonstrates effective verbal and written communication skills. * Takes initiative by asking others if they need assistance when they has downtime or taking the initiative to complete tasks that they are not assigned or cross training in other areas without the directive from their supervisor. * Maintains a clean, safe and organized work area at all times. * Ability to work with public in a friendly and courteous manner. * Follow food safety guidelines to promote safe food handling. * Demonstrates supportive skills in providing excellent customer service experiences. * Manages the process of the cash/receipt for any authorizations to release information. * Follow Administrative chain of command * Performs any other job-related duties as needed, assigned or requested * Follow SYHealth's personal appearance policy and avoid being put in the position of being sent home by supervisor. * Demonstrates a positive attitude that encourages motivation among peers and co-workers and promotes SYHealth's mission.

Job Requirements:

Education Required (Minimum level of education):

* High School Diploma or GED equivalent.

Certifications/Licenses Preferred:

* Current CPR Certification, Current Food Handler Card, and Current Driver License in the State of California or License from legal residence state. * Proof of vehicle registration and vehicle insurance. Clean driver's record. Must have reliable transportation and vehicle large enough to carry a large amount of coolers for Senior meals.

Experience Required (Minimum level of experience):

* Preferred: 1-2 years working with seniors

Verbal and Written Skills Required to perform the Job:

* Preferred: Bilingual (Spanish/English)

* Superb organizational ability, excellent customer service, compassionate, and exceptionally analytical.

Technical Knowledge and Skills Required to Perform the Job:

* Ability to proficiently maneuver through emails all electronic applications used for job.

Equipment Used:

* Computers, Software systems, kitchen appliances, Scanner, Telephones, Copy and Fax machines.

Working Conditions and Physical Requirements:

* Prolonged, extensive, and considerable standing/Sitting/Walking/Lifting. Lift up to 50 lbs * Manual dexterity and mobility. * Always reaching, stooping bending, kneeling, crouching. * Ability to handle multiple job task functions simultaneously. * Ability to work harmoniously with others as a team member. * May be required to work evenings and/or weekends, and provide staffing coverage.
